Chronic Disease Management (CDM) is a bi-annual check up offered to patients of qualifying age with any of the below medical conditions. At present CDM appointments are offered to those aged over 65. These check ups are covered by GMS and include fasting bloods and a urine sample check.
- Stroke / TIA
- Asthma
- COPD
- Diabetes (type 2)
- Ischaemic Heart Disease
- Atrial Fibrillation
- Heart Failure
At Imokilly Medical Centre we will send a letter of invitation to those patients that qualify for Chronic Disease Management if they have not attended the Practice within 6 months. If you have any of the conditions listed above, are over 65 and have not been to see one of our GPs within 6months please contact reception.
